Parent Initiated Therapy

Cannington Community Education Support Centre is committed to ensuring the best educational outcomes for our students. A minimum standard contract has been developed to promote a collaborative working relationship between Therapists, Parents, and CCESC.

Please read our Parent Therapy Process Letter and Letter to Therapy Agencies to better understand the CCESC therapy process, for in-school therapy sessions.

We invite all therapists to read the Department of Education Service Providers in Schools document. An on-site Induction will also be completed at school, once therapy services are approved. This helps us work together to create a safe and supportive environment for our students.

All therapists must provide the following before attending on-site:

  • Current Working with Children’s check (WWCC)
  • Professional Registration
  • Certificate of Currency (Public Liability to $20,000,000 – unlimited in the aggregate)
  • National Coordinated Criminal History Check (NCCHC) or NDIS clearance

Our school is co-located across two sites. Please click on the links below to download the documents relevant to the student and site you would like to visit:

Middle School – (Years 7 – 10)

Year 7 – 10 (Cannington Community College site) – 147 Wharf Street, Cannington

Parent/Carers Therapy Request – Middle School

Provider Therapy Service Schedule Agreement – Middle School

Senior School – (Years 11 – 12)

Parent/Carers Therapy Request – Senior School

Provider Therapy Service Schedule Agreement – Senior School

Each Therapist MUST complete a separate form, including two SMART (Specific, Measurable, Achievable, Realistic, Timely) goals that are relevant to school and the students’ education outcomes.

All documents must be returned by email to: Cannington.CESC.Therapy@education.wa.edu.au

Once all documents have been received, and approved, (make sure the correct documents are completed as this also delays processing), we can then begin to schedule therapy sessions at school.

Please note, due to the high volume of requests, this can take up to one week for processing to be completed.

We thank you for your patience.
